Our Occupational Therapy Home Safety Assessment looks beyond the physical environment to understand how you move, function, and complete everyday activities within your home. We evaluate the person, the activity, and the environment together to identify potential safety concerns and develop practical, personalized strategies to help you remain safe and independent at home.

A home may look safe, but everyday activities can become challenging as balance, strength, mobility, vision, sensation, or cognition change.

The Safe @ Home Assessment may be helpful if you or your loved one has:

  • Experienced a recent fall or near-fall

  • Started using a cane, walker, or other mobility device

  • Difficulty getting in or out of the shower or bathtub

  • Difficulty getting up from the toilet, bed, or chair

  • Increasing difficulty navigating stairs

  • Difficulty carrying items while walking

  • Changes in balance, strength, or coordination

  • Changes in memory, attention, or judgment

  • Difficulty safely completing everyday activities

  • Had a recent hospitalization or change in functional status

  • A caregiver or family member who is concerned about safety

  • A desire to proactively age in place

What happens at a Safe @ Home Assessment? 

1. We learn about YOU.

We begin by discussing your daily routine, concerns, goals, medical or functional changes, and what is most important to you.

3. We observe functional activities.

When appropriate, we observe you performing everyday activities such as transfers, walking, reaching, bathing-related tasks, stairs, or household activities.

2. We evaluate your space at home.

We walk through the areas of your home that are important to your daily activities and identify potential safety concerns.

4. You receive a Safe @ Home Plan

We prioritize recommendations based on your specific needs, goals, and level of risk.​ Your recommendations may include immediate safety changes, adaptive equipment, environmental modifications, and strategies for completing daily activities.
